    Baixos níveis séricos de vitamina D aumentam significativamente o risco de morte em idosos com fraturas de quadril : coorte prospectiva

    Objective: to evaluate the relationship between 25(OH)D3 levels and fatal outcome in patients over 60 years of age undergoing surgical repair of hip fractures. Methods: prospective cohort of patients undergoing surgical repair of hip fractures. At admission, 25(OH)D3 levels were measured, among other parameters. Patients were followed for at least 1 year, and incident mortality was recorded. Results: 209 patients were included in the study, with a mean age of 79.5 ± 7.6 years among survivors and 80.7 ± 8.2 years among those who died in the first postoperative year (p=0.346). The 25(OH)D3 levels of survivors were significantly higher than those of patients who died (p=0.003). After adjusting for confounding variables, 25(OH)D3 levels below 12.5ng/mL were significant risk factors regardless of mortality (adjusted OR: 7.6; 95% CI: 2.35 to 24.56). Conclusions: our data show that serum 25(OH)D3 levels below 12.5ng/mL significantly and independently increased the risk of mortality in the first year after surgical repair of low-energy hip fracture in patients over 60 years of age in the geographic region where this study was conducted. Low albumin also showed a significant association with mortality in these patients. All other factors had no significant associations.Métodos: coorte prospectiva de pacientes submetidos a correção cirúrgica de fraturas de quadril. À admissão, foram medidos os níveis de 25(OH)D3, entre outros parâmetros. Os pacientes foram acompanhados por, pelo menos, um ano e a mortalidade foi registrada. Resultados: foram incluídos 209 pacientes no estudo, com média de idade de 79,5 ± 7,6 anos entre os sobreviventes e 80,7 ± 8,2 anos entre os que morreram no primeiro ano de pós-operatório (p=0,346). Os níveis de 25(OH)D3 dos sobreviventes foram significativamente maiores do que os dos pacientes que morreram (p=0,003). Após o ajuste para variáveis de confusão, níveis de 25(OH)D3 abaixo de 12,5ng/mL foram fator de risco significativo, independentemente da mortalidade (OR ajustado 7,6; IC 95% 2,35 24,56). Conclusões: níveis séricos de 25(OH)D3 abaixo de 12,5ng/mL aumentaram significativa e independentemente o risco de morte no primeiro ano após o reparo cirúrgico de fratura de quadril de baixa energia em pacientes com mais de 60 anos de idade na região geográfica onde este estudo foi realizado. A albumina baixa também teve associação significativa com a mortalidade nesses pacientes. Todos os outros fatores não tiveram associações significativas

    Objectives: Describe the results of the red reflex test in full-term newborns, as well as identifyfactors associated with red reflex test outcome and compare hospital length of stay betweenpatients with inconclusive and normal red reflex test results.Methods: Descriptive cross-sectional study of the results of the red reflex test performed in atertiary hospital maternity unit between 2014 and 2018. A nested case-control study was alsoperformed to search for anthropometric, gestational, and neonatal variables associated withthe outcome of the red reflex test.Results: There were121 identified cases of inconclusive red reflex test in 11,833 newborns.Sixteen alterations were confirmed, four considered severe: two cases of congenital glaucoma,one of cataract, and one of coloboma. Mean birth weight (p = 0.04), length (p = 0.03), and headcircumference (p = 0.02) were lower in patients with inconclusive red reflex test; however with-out a relevant effect size (d = −0.21, −0.22, and −0.25, respectively).The proportion of white,mixed-race, and black patients was significantly different between the groups (p < 0.001), witha higher chance of inconclusive results in mixed-race (OR = 2.22) and black (OR = 3.37) patientswhen compared to whites. An inconclusive red reflex test led to an increase in hospital lengthof stay from 62 to 82 hours (p < 0.001). Conclusions: The red reflex test was able to identify four severe alterations in 11,833 newborns(0.03%). In the 121 newborns in which the red reflex test was classified as inconclusive, therewas a 20-hour increase in the hospital length of stay, but a severe alteration was confirmedin only 3.3% of them. Differences in red reflex between white, mixed-race, and black patientsshould be considered.Objetivos: Descrever os resultados do teste do reflexo vermelho em recém-nascidos a termo.Identificar fatores associados ao resultado do teste do reflexo vermelho e comparar o tempode internac ̧ão entre pacientes com teste do reflexo vermelho duvidoso e normal.Métodos: Estudo transversal descritivo dos resultados do teste do reflexo vermelho feito emmaternidade de hospital terciário entre 2014 e 2018. Foi ainda feito estudo de caso-controleaninhado para pesquisa de variáveis antropométricas, gestacionais e neonatais associadas aoresultado do teste do reflexo vermelho.Resultados: Foram identificados 121 casos de teste do reflexo vermelho duvidoso em 11.833recém-nascidos. Foram confirmadas 16 alterac ̧ões, 4 consideradas graves: 2 casos de glaucomacongênito, um de catarata e um de coloboma. As médias de peso de nascimento (p = 0,04), com-primento (p = 0,03) e perímetro cefálico (p = 0,02) foram menores nos pacientes com teste doreflexo vermelho duvidoso, entretanto sem um tamanho de efeito relevante (d = -0,21; -0,22e -0,25; respectivamente). A proporc ̧ão de pacientes brancos, pardos e negros foi estatisti-camente diferente entre os grupos (p < 0,001), com maior chance de resultado duvidoso parapardos (OR = 2,22) e negros (OR = 3,37) em comparac ̧ão a brancos. O teste do reflexo vermelhoduvidoso levou a um aumento no tempo de internac ̧ão de 62 para 82 horas (p < 0,001).Conclusões: O teste do reflexo vermelho foi capaz de identificar 4 alterac ̧ões graves em 11.833recém-nascidos (0,03%).     Efeito do comprimento axial ocular na espessura da camada de fibras nervosas da retina e da camada de células ganglionares-plexiforme interna avaliadas por OCT espectral

    Purpose: To evaluate the influence of ocular axial length on circumpapillary retinal nerve fiber layer and ganglion cell-inner plexiform layer thickness in healthy eyes after correcting for ocular magnification effect. Methods: In this cross-sectional study, we evaluated 120 eyes from 60 volunteer participants (myopes, emmetropes, and hyperopes). The thickness of the circumpapillary retinal nerve fiber layer and ganglion cell-inner plexiform layer were measured using the spectral optical coherence tomography (OCT)-Cirrus HD-OCT and correlated with ocular axial length. Adjustment for ocular magnification was performed by applying Littmann’s formula. Results: Before the adjustment for ocular magnification, age-adjusted mixed models analysis demonstrated a significant negative correlation between axial length and average circumpapillary retinal nerve fiber layer thickness (r=-0.43, p<0.001), inferior circumpapillary retinal nerve fiber layer thickness (r=-0.46, p<0.001), superior circumpapillary retinal nerve fiber layer thickness (r=-0.31, p<0.05), nasal circumpapillary retinal nerve fiber layer thickness (r=-0.35, p<0.001), and average ganglion cell-inner plexiform layer thickness (r=-0.35, p<0.05). However, after correcting for magnification effect, the results were considerably different, revealing only a positive correlation between axial length and temporal retinal nerve fiber layer thickness (r=0.42, p<0.001). . Additionally, we demonstrated a positive correlation between axial length and average ganglion cell-inner plexiform layer thickness (r=0.48, p<0.001). All other correlations were not found to be statistically significant. Conclusions: Before adjustment for ocular magnification, axial length was negatively correlated with circumpapillary retinal nerve fiber layer and ganglion cell-inner plexiform layer thickness measured by Cirrus-OCT. We attributed this effect to ocular magnification associated with greater axial lengths, which was corrected with the Littman’s formula. Further studies are required to investigate the impact of ocular magnification correction on the diagnostic accuracy of Cirrus-OCT.Objetivo: Avaliar a influência do comprimento axial ocular na espessura da camada de fibras nervosas da retina peripapilar e na espessura da camada de células ganglionares-plexiforme interna em olhos saudáveis após correção para efeito de magnificação ocular. Métodos: Neste estudo transversal, avaliamos 120 olhos de 60 participantes voluntários (míopes, emétropes e hipermétropes). A espessura da camada de fibras nervosas da retina peripapilar e da camada de células ganglionares-plexiforme interna foram medidas usando a tomografia de coerência óptica espectral (OCT)-Cirrus HD-OCT e correlacionada com o comprimento axial ocular. O ajuste para a magnificação ocular foi realizado aplicando a fórmula de Littmann. Resultados: Antes do ajuste para magnificação ocular, a análise de modelos mistos ajustada por idade demonstrou uma correlação negativa significante entre o comprimento axial e a espessura média da camada de fibras nervosas da retina peripapilar (r=-0,43; p<0,001), espessura da camada de fibras nervosas da retina peripapilar inferior (r=-0,46; p <0,001), espessura da camada de fibras nervosas da retina peripapilar superior (r=-0,31; p<0,05), espessura da camada de fibras nervosas da retina peripapilar nasal (r=-0,35; p<0,001) e espessura média das células ganglionares-plexiforme interna (r=-0,35; p<0,05). No entanto, após a correção do efeito de magnificação, os resultados foram consideravelmente diferentes, revelando apenas uma correlação positiva entre o comprimento axial e a espessura temporal da camada de fibras nervosas da retina (r=0,42; p<0,001). Além disso, demonstramos uma correlação positiva entre o comprimento axial e a espessura média das células ganglionares-plexiforme interna (r=0,48; p<0,001). Todas as outras correlações não foram consideradas estatisticamente significativas Conclusão: Antes do ajuste para o efeito de magnificação ocular, o comprimento axial estava negativamente correlacionado com a espessura da camada de fibras nervosas da retina peripapilar e das células ganglionares-plexiforme interna medido pelo Cirrus-OCT. Atribuimos esse efeito à magnificação ocular associada a comprimentos axiais maiores, o que foi corrigido com a fórmula de Littman. Mais estudos são necessários para investigar o impacto da correção da magnificação ocular na acurácia diagnóstica do Cirrus-OCT

    Program for healthier school cafeterias in Rio Grande do Sul, Brazil : protocol for a community-based randomized trial

    Background: School cafeterias can promote poor eating habits, as these retail outlets have a variety of foods considered to be nonnutritive and unhealthy. However, despite the need for effective preventive strategies, there is still disagreement on the best approach due to the lack of evidence on interventions to prevent and treat obesity in the school settings. Objective: We aim to verify the efficacy of an educational intervention program to improve the hygienic conditions and the composition of the menu offered in school cafeterias in the state of Rio Grande do Sul, Brazil. Methods: We will conduct a randomized, parallel, two-arm, community-based controlled study. Elementary and high schools, both public and private, in the State of Rio Grande do Sul, Brazil, that have a cafeteria will be eligible. Schools will be recruited and randomly assigned to the intervention (n=27) or control (n=27) group. The intervention group will receive an educational intervention program based on the guidelines issued by the Ministry of Health of Brazil, consisting of a 160-hour distance-learning qualification course, for 10 weeks, and using the Moodle platform and WhatsApp app. The intervention targets the owners and people in charge of the cafeterias, food handlers, principals, vice principals, teachers, pedagogical coordinators, dietitians, representatives of students' parents, and students over 16 years old. Meanwhile, the control group will receive only a printed copy of the book containing the guidelines used. The efficacy of the intervention will be determined by the hygienic conditions of the cafeteria and the composition of the menu offered, also considering the levels of processing of food sold. All outcomes will be analyzed as intention-to-treat and per-protocol. We will use covariance analysis or a generalized linear model for continuous data and ordinal logistic regression for ordinal categorical data. The level of statistical significance considered will be P<.05 for a 95% CI. Results: This project was funded in early 2018. We administered the intervention program in 2019. All data have already been collected, and we are analyzing the data. The results are expected in 2021. Conclusions: To our knowledge, this may be the first randomized controlled study in school cafeterias held in Brazil. The results will provide evidence for the formulation of public food and nutritional security policies and for the development of effective strategies to provide safe and healthy school meals

    Proposed Risk Score in Patients with Aortic Stenosis Submitted to Valve Replacement Surgery

    ABSTRACT Introduction: Due to Brazilian population aging, prevalence of aortic stenosis, and limited number of scores in literature, it is essential to develop risk scores adapted to our reality and created in the specific context of this disease. Methods: This is an observational historical cohort study with analysis of 802 aortic stenosis patients who underwent valve replacement at Hospital São Lucas, Pontifícia Universidade Católica do Rio Grande do Sul, from 1996 to 2018. With the aid of logistic regression, a weighted risk score was constructed based on the magnitude of the coeficients β of the logistic equation. Two performance statistics were obtained: area under the receiver operating characteristic curve and the chi-square (χ2) of Hosmer-Lemeshow goodness-of-fit with Pearson’s correlation coeficient between the observed events and predicted as a model calibration estimate. Results: The risk predictors that composed the score were valve replacement surgery combined with coronary artery bypass grafting, prior renal failure, New York Heart Association class III/IV heart failure, age > 70 years, and ejection fraction < 50%. The receiver operating characteristic curve area was 0.77 (95% confidence interval: 0.72-0.82); regarding the model calibration estimated between observed/predicted mortality, Hosmer-Lemeshow test χ2 = 3,70 (P=0.594) and Pearson’s coeficient r = 0.98 (P<0.001). Conclusion: We propose the creation of a simple score, adapted to the Brazilian reality, with good performance and which can be validated in other institutions

    Propensity-matched analysis of patients with intrahepatic cholangiocarcinoma or mixed hepatocellular-cholangiocarcinoma and hepatocellular carcinoma undergoing a liver transplant

    BACKGROUND Cholangiocarcinoma (CC) is a rare tumor that arises from the epithelium of the bile ducts. It is classified according to anatomic location as intrahepatic, perihilar, and distal. Intrahepatic CC (ICC) is rare in patients with cirrhosis due to causes other than primary sclerosing cholangitis. Mixed hepatocellular carcinoma-CC (HCC-CC) is a rare neoplasm that shows histologic findings of both HCC and ICC within the same tumor mass. Due to the difficulties in arriving at the correct diagnosis, patients eventually undergo liver transplantation (LT) with a presumptive diagnosis of HCC on imaging when, in fact, they have ICC or HCC-CC. AIM To evaluate the outcomes of patients with intrahepatic cholangiocarcinoma or mixed hepatocellular-cholangiocarcinoma on pathological examination after liver transplant. METHODS Propensity score matching was used to analyze tumor recurrence (TR), overall mortality (OM), and recurrence-free survival (RFS) in LT recipients with pathologically confirmed ICC or HCC-CC matched 1:8 to those with HCC. Progression-free survival and overall mortality rates were computed with the Kaplan-Meier method using Cox regression for comparison. RESULTS Of 475 HCC LT recipients, 1.7% had the diagnosis of ICC and 1.5% of HCC-CC on pathological examination of the explant. LT recipients with ICC had higher TR (46% vs 11%; P = 0.006), higher OM (63% vs 23%; P = 0.002), and lower RFS (38% vs 89%; P = 0.002) than those with HCC when matched for pretransplant tumor characteristics, as well as higher TR (46% vs 23%; P = 0.083), higher OM (63% vs 35%; P = 0.026), and lower RFS (38% vs 59%; P = 0.037) when matched for posttransplant tumor characteristics. Two pairings were performed to compare the outcomes of LT recipients with HCC-CC vs HCC. There was no significant difference between the outcomes in either pairing. CONCLUSION Patients with ICC had worse outcomes than patients undergoing LT for HCC. The outcomes of patients with HCC-CC did not differ significantly from those of patients with HCC

    Realizou-se um questionário fechado entre jovens universitários, de 16 aos 22 anos, da Pontifícia Universidade Católica e da Universidade Federal do Rio Grande do Sul, com o objetivo de avaliar o comportamento sexual da juventude universitária de Porto Alegre. Obteve-se 682 inqéritos, compreendendo 58,2% do sexo feminino e 41,8% do sexo masculino. Foram levantados os índices de prevalência acerca dos aspectos que costumam ser mais importantes e necessários de se tornarem conhecidos. Encontrou-se a prática da masturbacão em 77, 1%, relacões sexuais em 70,8% e a realização do aborto em 13,2%. Também discuÍiu-se a idade da primeira relação sexual, o companheiro escolhido para iniciação, o conhecimento dos pais a respeito das relações pré-matrimoniais, o local preferido para as relações, o conhecimento dos métodos anticoncepcionais, sua utilização, as informações a respeito das doenças sexualmente transmissíveis, a homossexualidade, o sexo em grupo e a necessidade de afeto nas relações sexuais.682 university students, aged 16 to 22, from the Pontifícia Universidade Catolica an the Federal University of Rio Gra~;~de do Sul responded to close-questions related to sexual behavior, 58.2% were females; 41.8% males.     The prevalence and adequacy of antibiotic prescriptions in Hospital de Clínicas de Porto Alegre

    Durante o dia 30 de julho de 1987, foram estudados alguns aspectos envolvendo o uso de antibióticos no Hospital de Clínicas de Porto Alegre. De 432 pacientes internados, 182 (42%) estavam utilizando antibióticos,e destes, 42% recebiam a medicação de forma inadequada. Dos antibióticos usados terapeuticarhente, os mais prescritos foram ampicilina, penicilina, cefalosporinas e amicacina. Entre os usados profilaticamente, destacaram-se cefa/osporinas, gentamicina e ampicilina. O trabalho sugere que o controle do uso de antibióticos pode reduzir a adoção inapropriada destes agentes.On July 30th 1987 there was made a survey to evaluate the antibiotic use in the Hospital de Clínicas de Porto Alegre. 432 patients were investigated and 182 (42%) were under antimicrobial therapy. This group showed 42% o f patients receiving antimicrobians inappropriately. The antibiotics most used therapeuticaly were ampicillin, penicillin, cefalosporin and amikacin. For prophylaxis the most used were cefalosporin, gentamicin and ampicillin. This paper suggests that contra/ procedures can reduce the inappropriate use of antimicrobial agents in our hospital

    Structural brain abnormalities in the common epilepsies assessed in a worldwide ENIGMA study

    Progressive functional decline in the epilepsies is largely unexplained. We formed the ENIGMA-Epilepsy consortium to understand factors that influence brain measures in epilepsy, pooling data from 24 research centres in 14 countries across Europe, North and South America, Asia, and Australia. Structural brain measures were extracted from MRI brain scans across 2149 individuals with epilepsy, divided into four epilepsy subgroups including idiopathic generalized epilepsies (n =367), mesial temporal lobe epilepsies with hippocampal sclerosis (MTLE; left, n = 415; right, n = 339), and all other epilepsies in aggregate (n = 1026), and compared to 1727 matched healthy controls. We ranked brain structures in order of greatest differences between patients and controls, by meta-Analysing effect sizes across 16 subcortical and 68 cortical brain regions. We also tested effects of duration of disease, age at onset, and age-by-diagnosis interactions on structural measures. We observed widespread patterns of altered subcortical volume and reduced cortical grey matter thickness. Compared to controls, all epilepsy groups showed lower volume in the right thalamus (Cohen's d = \uc3\ua2 '0.24 to \uc3\ua2 '0.73; P < 1.49 \uc3\u97 10 \uc3\ua2 '4), and lower thickness in the precentral gyri bilaterally (d = \uc3\ua2 '0.34 to \uc3\ua2 '0.52; P < 4.31 \uc3\u97 10 \uc3\ua2 '6). Both MTLE subgroups showed profound volume reduction in the ipsilateral hippocampus (d = \uc3\ua2 '1.73 to \uc3\ua2 '1.91, P < 1.4 \uc3\u97 10 \uc3\ua2 '19), and lower thickness in extrahippocampal cortical regions, including the precentral and paracentral gyri, compared to controls (d = \uc3\ua2 '0.36 to \uc3\ua2 '0.52; P < 1.49 \uc3\u97 10 \uc3\ua2 '4). Thickness differences of the ipsilateral temporopolar, parahippocampal, entorhinal, and fusiform gyri, contralateral pars triangularis, and bilateral precuneus, superior frontal and caudal middle frontal gyri were observed in left, but not right, MTLE (d = \uc3\ua2 '0.29 to \uc3\ua2 '0.54; P < 1.49 \uc3\u97 10 \uc3\ua2 '4). Contrastingly, thickness differences of the ipsilateral pars opercularis, and contralateral transverse temporal gyrus, were observed in right, but not left, MTLE (d = \uc3\ua2 '0.27 to \uc3\ua2 '0.51; P < 1.49 \uc3\u97 10 \uc3\ua2 '4). Lower subcortical volume and cortical thickness associated with a longer duration of epilepsy in the all-epilepsies, all-other-epilepsies, and right MTLE groups (beta, b < \uc3\ua2 '0.0018; P < 1.49 \uc3\u97 10 \uc3\ua2 '4). In the largest neuroimaging study of epilepsy to date, we provide information on the common epilepsies that could not be realistically acquired in any other way. Our study provides a robust ranking of brain measures that can be further targeted for study in genetic and neuropathological studies. This worldwide initiative identifies patterns of shared grey matter reduction across epilepsy syndromes, and distinctive abnormalities between epilepsy syndromes, which inform our understanding of epilepsy as a network disorder, and indicate that certain epilepsy syndromes involve more widespread structural compromise than previously assumed

    Differential cross section measurements for the production of a W boson in association with jets in proton–proton collisions at √s = 7 TeV

    Measurements are reported of differential cross sections for the production of a W boson, which decays into a muon and a neutrino, in association with jets, as a function of several variables, including the transverse momenta (pT) and pseudorapidities of the four leading jets, the scalar sum of jet transverse momenta (HT), and the difference in azimuthal angle between the directions of each jet and the muon. The data sample of pp collisions at a centre-of-mass energy of 7 TeV was collected with the CMS detector at the LHC and corresponds to an integrated luminosity of 5.0 fb[superscript −1]. The measured cross sections are compared to predictions from Monte Carlo generators, MadGraph + pythia and sherpa, and to next-to-leading-order calculations from BlackHat + sherpa. The differential cross sections are found to be in agreement with the predictions, apart from the pT distributions of the leading jets at high pT values, the distributions of the HT at high-HT and low jet multiplicity, and the distribution of the difference in azimuthal angle between the leading jet and the muon at low values.United States.
